Diagnosis worldwide should be based on the International Classification of Headache Disorders, 2^nd edition (ICHD-II), but this needs simplification and reduction to essentials to be useful in primary care.

Lifting The Burden has produced a core cut-down version of ICHD-II. Region-specific variations will be developed for use around the world.

Diagnostic algorithms would be unnecessary if educational initiatives were successful and reached primary-care physicians everywhere, but this will never be achieved.

Diaries, for completion over periods of time by individual patients, are informative for diagnosis and for outcome evaluation. The same diary does not ideally serve both purposes. Many diaries of each type exist, but the optimal design is unknown.

Lifting The Burden is collaborating with a working group of the EuroHead project to produce and test a diagnostic headache diary.
